Palliative Care
When patients are facing a serious illness, Palliative Care offers services that address the most pressing needs today, while also planning for tomorrow – helping to maintain independence. The team helps patients and their caregivers, along with their primary physician to manage pain and other distressing symptoms that can make each day a challenge. They also connect those who are facing a chronic and or/terminal condition with education, support and community resources. Bringing support alongside patients/working together to keep patients safe.
Palliative care is specialized medical care for people with chronic illnesses. It focuses on providing patients with relief from the symptoms, pain, physical stress and mental stress of a serious illness, whatever the diagnosis. The goal of such therapy is to improve quality of life for both the patient and the family.
Vanessa has been the Palliative Pathways Outreach Coordinator for Hospice of the West since mid-2017. Vanessa graduated from Arizona State University with a Bachelor’s degree in social work. She has been working as a hospice and palliative care social worker for over 15 years. She is currently a member of the Thoughtful Life Conversations Committee which empowers Arizonans to make their life wishes and care directives known. She has a passion for advocating for patients and encouraging open communication regarding end of life planning.
Respite Care
We may be able to provide free respite care for those with Alzheimer’s/Dementia during the caregiver meeting. This will allow the caregiver the opportunity and peace of mind to attend the discussion group. Please call me with your RSVP no later than Tuesday, May 15.
Monthly Breakfast
A more casual opportunity to meet and share with other caregivers is the first Saturday of each month. We will meet at Eagle’s Nest for breakfast and just talk. Getting to know other caregivers can be very supportive and helpful as the journey of this disease can be challenging. RSVP is required by May 1 for the May 4 breakfast.
